Dermatology, by means of aesthetic facial procedures, maximizes the outcomes of plastic surgery.

 

Nowadays, there are several aesthetic procedures of unquestionable effectiveness for facial rejuvenation. The incorporation of such techniques to the surgical plan may maximize the quality and the durability of the outcomes achieved by the surgery, offering even higher levels of satisfaction to the patient and the doctor. The following procedures may be performed by our team:
 


Peeling
Peeling consists basically in the application of a certain substance or energy that Will provide the renovation of the skin. Regardless of the technique, the aim is to replace the aged-looking skin due to the presence of thin wrinkles and stains with a “new”, rejuvenated-looking skin with smoother texture. Unfortunatelly, peeling has no relaxing effect on the facial musculature. Thus, patients with expression wrinkles, caused by the contraction of the underlying muscles, should not be treated by this method.
 


In facial surgery, the execution of peeling on the regions around the eyes, the mouth or even the whole face may significantly improve the outcome. Due to the excellent outcomes with high levels of safety, we recommend the techniques that use special acids or superficial laser peeling. Such treatments may be performed during the surgery and need specific postoperative cares, especially regarding creams and protection from the sun.
 


Dermabrasion
Dermoabrasion is a procedure that uses high-rotation sandpapers to perform a more profound peeling than those which use acids or laser. For it can be made during surgery, such technique offers great outcomes to patients with thin wrinkles around the mouth. In patients with scars resulting from acne, great outcomes may be obtained by a profound renovation of the skin, which can be made in many stages.  Patients who undergo dermabrasion also need specific postoperative cares, especially regarding application of creams and protection from the sun.
 


Fat Graftings
Fat graftings have been even more often used in surgery of the face, since the process of aging on this region necessarily involves the atrophy of the tissues, resulting in greater visibility of the underlying osseous structure. Therefore, the restitution of the tissular volume employing fat from other body parts may help to soften the changes in the facial outline that occur along time, especially in the furrow between the mouth and the cheeks and in the lips. It is important to remind that about 30-50% of the fat grafted may be reabsorbed along the early months.  Anyway, the effects are permanent, since the fat grafted is part of the very body and blends with the recipient site.
 


Chin Implant
A silicone implant in the region of the chin may be necessary to harmonize the aesthetic units of the face. Actually, the outline of the chin has a great influence on the profile of the face, along with the nose. Thus, this technique offers great outcomes when performed simultaneously with rhinoplasty. Currently, the implant may be performed through an intraoral incision or a small incision under the chin. Regardless of the way of access, the aim is to augment the projection of the chin, balancing it with the rest of the face.
 


Filling
The use of filling materials may offer good outcomes to patients with early signs of aging, to those who don’t want to undergo surgery, and as a complement after surgical treatment. Basically, we recommend the use of this technique for patients who bear repose wrinkles – those which are present without contraction of the facial muscles. The most common regions are the furrow between the mouth and the cheeks, the region between the eyebrows, and the thin wrinkles around the lips. It is worth reminding that expression wrinkles, caused by the contraction of the underlying muscles, must be treated by paralysis of such muscles with botulinum toxin. Due to the low rate of complications, we recommend the use of absorbable substances such as hyaluronic acid. The effects of the treatment last about 6-8 months and can be repeated after this period.
 


Botulinum Toxin
Botulinum toxin has been employed with great outcomes to soften expression wrinkles on the forehead and around the eyes. This technique may also offer good results to patients with early signs of aging, to those who don’t want to undergo surgery, and as a complement after surgical treatment. Produced by bacteria  Clostridium botulinum type A, the toxin is highly purified and offers no risk to the patient when the application rules and volumes are strictly obeyed.
 


The mechanism of action of botulinum toxin consists in the paralysis of the muscles that, due to their tight adherences to the skin, produce wrinkles during their contraction. It is worth reminding that thin wrinkles caused by changes in the texture of the skin due to sun exposure and natural aging cannot be leavened by botulinum toxin. In these cases, we recommend peeling. Applied under local anesthesia, botulinum toxin presents its higher effect after about 7 days, being the typical duration of 4-6 months. Just like filling, the application may be repeated along the years.
